Privacy Policy

Information Collection and Use

While using our Service, we may ask you to provide us with certain personally identifiable information that can be used to contact or identify you. Personally Identifiable Information may include, but is not limited to, your email address, name, phone number, postal address, other information (“Personal Information”).

We collect this information for the purpose of providing the Service, identifying and communicating with you, responding to your requests/inquiries, servicing your purchase orders, and improving our services.

Public Forum

This website may include public forums such as message boards, bulletin boards, chat rooms, space for commenting (such as on blog posts), or similar activities through which users can communicate with one another after logging in through a secure network. This privacy policy does not protect you when you use public forums or provide information, including Personally Identifiable Information (PII), about yourself. You should be aware that any information shared in a public forum is public information and may be seen by third parties that do not adhere to our Privacy Policy.

We urge you to exercise discretion and caution when deciding to disclose your Protected Health Information (PHI) regarding your personal health information, or any other information, through the forum. Further, we are not responsible for the content of any messages posted in a forum capacity, and we retain the right to remove comments from our website at any time and without notice.

Log Data

We may also collect information that your browser sends whenever you visit our Service (“Log Data”). This Log Data may include information such as your computer’s Internet Protocol (“IP”) address, browser type, browser version, the pages of our Service that you visit, the time and date of your visit, the time spent on those pages and other statistics. In addition, we may use third party services such as Google Analytics that collect, monitor and analyze this type of information in order to increase our Service’s functionality. These third-party service providers have their own privacy policies addressing how they use such information.

Cookies are files with a small amount of data, which may include an anonymous unique identifier. Cookies are sent to your browser from a web site and transferred to your device. We use cookies to collect information in order to improve our services for you.

Behavioral Remarketing

Unity Recovery Center uses re-marketing services to advertise on third party web sites to you after you visit our Service. We, and our third-party vendors, use cookies to inform, optimize and serve ads based on your past visits to our Service.

Google

Google AdWords remarketing service is provided by Google Inc.

You can opt-out of Google Analytics for Display Advertising and customize the Google Display Network ads by visiting the Google Ads Settings page.

Do Not Track Disclosure

We support Do Not Track (“DNT”). Do Not Track is a preference you can set in your web browser to inform websites that you do not want to be tracked. You can enable or disable Do Not Track by visiting the Preferences or Settings page of your web browser.

We may employ third party companies and individuals to facilitate our Service, to provide the Service on our behalf, to perform Service-related services and/or to assist us in analyzing how our Service is used. These third parties have access to your Personal Information only to perform specific tasks on our behalf and are obligated not to disclose or use your information for any other purpose.

The security of your Personal Information is important to us, and we strive to implement and maintain reasonable, commercially acceptable security procedures and practices appropriate to the nature of the information we store, in order to protect it from unauthorized access, destruction, use, modification, or disclosure.

However, please be aware that no method of transmission over the internet, or method of electronic storage is 100% secure and we are unable to guarantee the absolute security of the Personal Information we have collected from you.

Your information, including Personal Information, may be transferred to — and maintained on — computers located outside of your state, province, country or other governmental jurisdiction where the data protection laws may differ than those from your jurisdiction.

If you are located outside United States and choose to provide information to us, please note that we transfer the information, including Personal Information, to United States and process it there.

Your consent to this Privacy Policy followed by your submission of such information represents your agreement to that transfer.

Links to Other Sites

Our Service may contain links to other sites that are not operated by us. If you click on a third-party link, you will be directed to that third party’s site. We strongly advise you to review the Privacy Policy of every site you visit.

We have no control over, and assume no responsibility for the content, privacy policies or practices of any third-party sites or services.

Embedded content from other websites

Articles on this site may include embedded content (e.g. videos, images, articles, etc.). Embedded content from other websites behaves in the exact same way as if the visitor has visited the other website.

Children’s Privacy / Children Online Privacy Protection Act (COPPA)

Only persons age 18 or older have permission to access our Service. Our Service does not address anyone under the age of 13 (“Children”).

We do not knowingly collect personally identifiable information from children under 13. If you are a parent or guardian and you learn that your Children have provided us with Personal Information, please contact us. If we become aware that we have collected Personal Information from children under age 13 without verification of parental consent, we take steps to remove that information from our servers.

Who We Are

This notice describes the privacy practices of Unity Recovery Center.

Our Privacy and Confidentiality Obligations

We are required by law to maintain the privacy and confidentiality of information about your health, health care, and payment for services related to your health and to provide you with this notice of our legal duties and privacy practices with respect to your protected health information. When we use or disclose this information, we are required to abide by the terms of this Notice or any other public notice in effect at the time of the use or disclosure.

  • Protected Health Information in connection with alcohol or drug services:
    • 42 CFR Part 2 protects your health information if you are applying for or receiving services (including diagnosis or treatment, or referral) for drug or alcohol abuse. Generally, if you are applying for or receiving services for drug or alcohol abuse, we may not acknowledge to a person outside the program that you attend the program or disclose any information identifying you as an alcohol or drug abuser except under certain circumstances that are listed in this notice.
  • All Protected Health Information, including alcohol or drug services:
    • The Health Insurance Portability and Accountability Act, also known as HIPAA, 45 CFR Parts 160 and 164 protects your health information whether or not you are applying for or receiving services for drug or alcohol abuse.

Who Will Follow This Notice?

This notice describes the information privacy practices followed by our employees, staff and other personnel.

Your Health Information

As per 45 CFR 164.520, this Notice of Privacy Practices (the Notice) describes how medical information about you may be used or disclosed and how you can access this information.

Your personal health record contains private and confidential information about you and your health. Both State and Federal laws protect the confidentiality of this information.

Protected Health Information (PHI) under the Health Insurance Portability and Accountability Act of 1996 (HIPAA) includes any individually identifiable health information.

PHI relates to your past, present or future physical or mental health or condition and any related health care services.

How We May Use and Disclose Health Information About You

Below are examples of the uses and disclosures that we may make of your Protected Health Information (PHI). These examples are not exhaustive but simply describe the uses and disclosures that may be made. Uses and Disclosures of PHI for Treatment, Payment, and Health Care Operations

Treatment

Your PHI (Protected Health Information) may be used and disclosed by your physician, counselor, program staff, and others involved in your care for purposes such as:

  • Providing, coordinating, or managing your health care treatment.
  • Consulting with other providers.
  • Referrals to other health care providers.

Example: We may disclose PHI to another physician or provider involved in your care.

Payment

With your authorization, we may use and disclose your PHI to:

  • Receive payment for treatment and services provided.
  • Communicate with insurance providers or payors.

Example: Your insurance company may receive necessary information to process claims.

Health Care Operations

We may use and share your PHI to:

  • Run our business and improve your care.
  • Perform quality assessment and staff evaluations.
  • Comply with licensing requirements.

Examples include:

  • Use of sign-in sheets with your name and assigned provider.
  • Contacting you with appointment reminders or health information.
  • Sharing PHI with a billing company or other administrative service providers.

Special Rules Regarding Behavioral Health, Substance Abuse, and HIV-Related Information

  • HIV-Related Information: May be disclosed as required or permitted by state law, such as for treatment, health oversight, or exposure notifications.

Other Uses and Disclosures That Do Not Require Your Authorization

Required by Law

  • Disclosures required by law or legal obligation.
  • You will be notified as required when disclosures occur.
  • We must provide PHI to you upon request and to the Department of Health and Human Services for compliance audits.

Health Oversight

Disclosures may be made to government agencies for:

  • Audits
  • Investigations
  • Licensing reviews

Note: Safeguards are required in these disclosures.

Public Health

Disclosures may be made to public health authorities to:

  • Prevent or control disease or injury.
  • Notify individuals potentially exposed to communicable diseases.

Medical Emergencies

PHI may be disclosed to medical personnel in emergencies. You will receive a copy of this notice post-emergency

Child Abuse or Neglect

Disclosures may be made to authorized agencies for:

  • Reporting suspected child abuse or neglect (limited to required information only).

Deceased Patients

PHI may be disclosed to:

  • Determine cause of death.
  • Comply with vital records laws.

Research

PHI may be used in research only if:

  • Approved by an Institutional Review Board.
  • Privacy safeguards are in place.
  • Researchers agree not to re-disclose your PHI.

Criminal Activity on Program Premises

We may disclose PHI to law enforcement for:

  • Crimes committed on-site or against staff.

Court Orders

Disclosures may occur with a valid court order that complies with federal and state law.=

Interagency Disclosures

Limited PHI may be shared among government agencies for service coordination.

Public Safety

In certain mental health settings, PHI may be disclosed to:

  • Prevent serious threats to health/safety.

Specialized Government Functions

If applicable, disclosures may occur for:

  • Military command authorities.
  • National security or intelligence purposes.
  • U.S. Department of State evaluations.

Family and Friends

PHI may be disclosed to individuals involved in your care:

  • With your verbal consent or implied agreement.
  • When you are present or based on professional judgment.
  • In emergencies when you are unable to consent.

Uses and Disclosures That Require Written Authorization

Written authorization is required for:

  • Marketing: Use of PHI for marketing purposes.
  • Sale of PHI: We will not sell your PHI without your signed authorization.
  • Psychotherapy Notes: Unless for treatment or legal defense.

You may revoke your authorization at any time in writing. However:

  • Past disclosures made under your authorization cannot be undone.
  • We are required to retain records of the care we provided.

Your Rights Regarding Protected Health Information

You have the right to:

  • Access and obtain a copy of your PHI.
  • Request an amendment to your PHI.
  • Receive an accounting of disclosures.
  • Request restrictions on certain uses or disclosures.
  • Request confidential communication methods.
  • File a complaint if you believe your rights have been violated.
